Voting Question: Anyone know some Federal tax questions? Related to combined disability, unemployment, and regular income...?

A friend worked only 11 weeks during early 2009 earning just over ten grand. She then had a serious knee operation and went onto puny company funded 710 dollars a month short term disability for 6 months. finally, she got some unemployment payments for a few months. This was in NY State. Of course, she had a lot of out of pocket med costs for MD visits, tests, physical therapy, travel, phone, other medical payments which total up, plus some other deductibles if she files the long form with miscellaneous deductions... in NY.
How does all this figure on both Federal tax returns as well as NY State tax dept. returns? Does the unemployment insurance count as regular income dollar for dollar....what about the short term disability payments? Any Special things and forms to do/use?

I have gotten a few different answers on this and its just confusing me. I am going to be receiving NY state short term disability insurance for my maternity leave. Does that start from the day I actually go into the hospital to have the baby then 6 weeks after? if I go out before the day I go into labor then do I not get paid unless I have a doctors note?Then 2nd part to that question if someone knows the answer, if I do have a doctors note to go out early would my 6 weeks start then or would I get paid for whatever time I went out early with doctors note plus the 6 weeks? I have looked online for this answer but of course NY state websites are not too helpful at all. I am going to meet with my HR rep at work next week but its been making me anxious lately not knowing about this and I only have 3 weeks to go. Worried about going early and not knowing how this works. PLEASE only answer if you are sure, no guessing

I am 42 y/o. I have a bad history of traumatic accidents throughout my life. At age 4 I was hit by a car and broke both legs and fractured my skull. I miraculously recovered from these injuries and went on to a somewhat normal childhood. I say 'somewhat' because at the time 1972, ptsd was not well defined or know about, at least I don't think it was because I recieved no psychological attention at the time. I do remember that starting school for me was really, REALLY a hard thing to do. Later on at age 16, a friend of min at the time asked me to go for a ride on his motorcycle. I was a minor at the time and didn't know about anything such as if he had a legal license or if the motorcycle was insured. It turned out he didn't have a licence and it wasn't insured and he crashed and I shattered my ankle and have nuerological problems with my foot, hammer toes.. a total nightmare. I recovered from this, but my foot has never been the same, still have hammer toes.. (what's it like to wear sandals, anyone?). Being that we were renting an apartment from the friend whose motorcycle it was my family told me not to sue the home insurance despite my attorney at the time saying we could. The lawsuit for my injuries was 3 million dollars and when the family got that letter they just about took out shotguns and threatened us. My mother and father then advised me not to sue them so I got no money for my injuries at all. Now at the age of 42 I have chronic back pain, foot pain, and I haven't been able to work since two other bad accidents in 1999 and 2002. My back is so bad that any stress or strain on it I am out of commission for sometimes more than 2 weeks. My doctor has been pumping me up on narcotics, oxycontin, oxycodone, etc.. but the doctor's reports never seem to get me disability benefits so my wife and I live in poverty, the 2002 accident happened while I was at work and workmans comp settled my claim for $500/mo. so my annual salary is $6,000.00 per year and this is all I make. My wife who is from Estonia, is illegal and we haven't had the money it would take to get her legal so we are both living in a small studio apartment and we can't get anyone to help our situation and have been living like this for 10 years. I really feel bad, I have no life, no way to make a living and no help from any agency. I have Binder & Binder but they didn't get me anything other than the usual 'Denial' letter from NY State Disability. I have been in a cycle of appeals for 10 years. I can't help but think that there are some people in prison who are living better than me, a regular guy who has been in accident's and is now just stuck like this.
Can anyone help us or advise us?

Any suggestions of how to get medical treatment for right arm numbness?
I broke my arm at work seven years ago. I had surgery and it has been fine with a mal union and inoperable joint that cracked into three pieces from a fixator placed incorrectly) until about two months ago- I went to a family doctor three times; he prescribed an anti anxiety med and sent me to the ER. I have progressive numbness, shooting pain from my elbow to my wrist and occasionally to my neck.
My workman’s comp Dr dropped me because he does not want to deal with NY workman’s comp insurance (He is in another state and took it originally out of pity for Starting in March,
In the ER three I was worked up for chest pain each time and assured my heart is great. My arm aches so bad that I cannot sleep in a bed and I take naproxen to manage the pain but is causing bruising and bleeding. I found a workman’s comp doc and had to wait a month to see him. Today I saw him. He asked for my regular insurance and stated he does not accept that company and asked if I would pay out of pocket for any portion of the bill workman’s comp does not pay. (I know this is illegal, but in NY illegal activities are not enforceable).
I called the comp board today and was told that my case states my original disability was limited to just the wrist so that ortho doc would not get reimbursed for my arm pain, and I need a hearing to correct it because all my original medical visits clearly state I originally broke my arm.
So my family doc keep sending me to the ER, the ER only does tests only for my heart then tells me to see a workman’s comp ortho doc. Local ortho doc’s do not want my case because it is much paperwork for little pay. One ortho doc saw me but will not do tests or treat me because he feels he will not get paid enough. My attorney needs to call a new hearing to correct the disability title.
The pain and numbness is do bad, I have digressed to crying frequently, the pain & numbness is awful. I cannot get a full nights sleep and when I take the pain meds during the day, I fall asleep for an hour.
Is there anyway to get treatment

I already have a ton of medical bills including an ambulance transport to rule out a heart attack. I can't afford any more incompetent doctors following protocals to get as much money as possible without any concern for treating my actually problem. These hospitals balance bill, so no matter what my insurnace pays, they have billed me at a higher rate that allows them to come after me after being paid by my insurance company.
The problem with workman's comp is that the doctors cannot balance bill and cannot collect additional money using inpatient-outpatient/negociated rates/ multi billing tactics.

Here is the scenario...Mom and teen son who live with a man( roommate), have been told the room mate is leaving because he cannot afford the rent anymore. (mom pays all the bills, plus all household expenses, food, cooks and takes care of her own bills like car insurance and teenager exposes.). Apt office won't accept another room mate to finish the lease.
Mom has three other options, but none of them are "for sure" or solid at the moment:
1, Live with a man who's known her 7 yrs-but it's a two family house and his aunt owns it and lives upstairs-she;s 85. He also has a 21 yr old daughter there full time, and 17 yr old son, all summer, all weekends and 4 hours everyday, after school.
She really loves this man, and they met because my dad married the lady that left this man. So they are the "x's".But the house is small and very cluttered and we fear the aunt will say no.
2 there is a possible ( maybe very remote, not sure yet), that a man who heard of the situation, might want to help and get an apt here in town.He's met with mom, and is 10 yrs older, a widower, and seems very serious about offering a place to live here, while he still maintains his own house he rents in NY. Nice older man.
3a friend she's not seenfor years, male, could possibly take her, but it's out of state. They have been telephone friends mostly.
She does not want to end up in some homeless shelter.
She went to welfare( she is on SSI) social security disability),and she was told to wait for an eviction notice and then they will put her and son in a shelter,, and in that time, they will "help" with rent, if she finds an apartment. There is no section 8 here.
What would YOU do?

am currently living in Burlington VT and on disability and medicaid due to a motor neuron disease (ALS) I am going to be moving to New York State in June to be closer to family. Will this affect my ability to still receive medicaid? I currently live with my fiance' and 3 children and receive constant medical care due to my condition. When in NY...i will be living with my fiance' and 3 kids as well...so the living arrangements (household income) will be the same, but just in a different state. Any feedback on this would be appreciated..Thanks! :)

I was injured on the job and am now seeking workers compensation. The accident took place in Oct. of '08 and I have a WC hearing coming up pretty soon. However, right now I should be receiving Disability, but the insurance company cannot begin making payments until my former employer completes their portion of the DB450 claim form. The form was first sent to them at the end of Oct. and the director only answered 3 of the 17 questions (he did not put my wages which is the single most important thing the insurance company needs). I do not wish to have any contact with the employer because they used false allegations to fire me on the day that I asked for the WC forms and I do want my contacting them to be an opportunity to make any more. Is it legal for them not to properly fill out this form? It seems spiteful and wrong. They also never did pay overtime and now it seems as if they have something to hide. Any suggestions>

My disability insurance is only paying for 6 weeks
after my c-section. My doctor says I should get 8 weeks. Does anyone know of a law in new york state that might support this? I would like to appeal their decision.

Resolved Question: Do I need a Workman's Comp Attorney?

I am running a small - medium size healthcare staffing company that provides LPNs and RNs to Nursing Homes in NY Metro area. Each nurse works as a 1099. One of the requirements is that before they can start on their assignments with our clients, they are required to fill out an Independent Contractor's agreement that states they are responsible for their own worker's Comp and Disability Insurance. 6 Months ago, while on an assignment, a nurse fell at the client's facility on the wet (from rain) floor and hurt her leg. She Then proceeded to file a Worker's Comp claim through the client. After I learned of this, I spoke to the client and the client agreed to put the claim through his facility's insurance (WC). Now I am getting a letter from Worker's Comp asking me how how my company has no Worker's Comp Ins. Should I be worried? If yes, do I need a specialized attorney? Do you know a good one in NY area?
